When applying for insurance compensation to ERV Travel Insurance JSC, the following documents must be provided:

Request (application) for payment of insurance compensation, which indicates:

• last name, first name and patronymic of the tourist, as well as information about another customer, if the contract for the sale of a tourist product was concluded by the customer;
• date of issue, validity period and other details of tour operator civil liability insurance contracts:

1) AGREEMENT No. GOTO-087/16 of civil liability insurance for non-fulfillment or improper fulfillment of obligations under the contract for the sale of a tourism product dated December 20, 2016;

2) AGREEMENT No. GOTO-023/17 for insurance of civil liability of a tour operator for failure to fulfill obligations under the contract for the sale of a tourist product dated April 04, 2017;

• number of the contract for the sale of a tourism product and the date of its conclusion;
• name of the tour operator who concluded the Insurance Agreement;
• information about circumstances (facts) indicating the tour operator’s failure to fulfill its obligations under the contract for the sale of a tourism product;
• the amount of actual damage caused to the Beneficiary in connection with the failure of the Insured to fulfill its obligations under the contract for the sale of a tourism product;
• information about applying for payment to other organizations that have provided the tour operator with financial security for the tour operator’s liability, as well as the order of payment by the organizations that have provided the tour operator with financial security for the tour operator’s liability.

The following documents are attached to the request (application):

• a copy of the passport (page spreads with photo and registration address) of the Beneficiary or other identification document in accordance with the legislation of the Russian Federation;
• a copy of the agreement on the sale of a tourism product, including all annexes and additional agreements to it (with presentation of its original) in case of concluding an agreement on the sale of a tourism product on paper;
• documents confirming the actual damage incurred by the Beneficiary as a result of the Insured's failure to fulfill obligations under the contract for the sale of a tourism product (original documents confirming payment under the contract for the sale of a tourism product, and the form filled out by the travel agency confirming the actual damage incurred by the beneficiary);
• to receive insurance compensation by a third party, it is necessary to provide an appropriate notarized power of attorney from the tourist and (or) other customer of the tourism product;
• in the event of an unexpected departure (evacuation) from the country (place) of temporary stay, in addition to the above list of documents, copies of the pages of the international passport of all trip participants (with personal information and marks on crossing the border of the Russian Federation), as well as documents confirming additional expenses, are provided;
